脊椎手術在許多民眾心中是一個巨大的讓人很惶恐的、可怕的、似乎是容易造成下半身不遂的手術,然而隨著醫療科技的日新月異,技術發展日漸純熟,在診間,這些舊有的觀念仍存在大家心中不易改變,脊椎狹窄的治療以大眾可區分為單純減壓與釘棒融合,病人與家屬要在短時間內完全瞭解有何不同,又該如何選擇有其難度。因此,我們團隊希冀藉由此醫病共享決策(SDM)的方式,經由與醫療團隊的討論與解釋,讓家屬與病人了解自己手術的需求之處。
退化性脊椎疾病是一個常見的問題。起因於脊椎的退化老化,俗稱長骨刺,因脊椎關節發生關節炎,骨刺增生或滑脫,使得椎管內的脊髓神經受到擠壓,造成所謂的脊椎坐骨神經痛。脊椎的退化性病變大多可保守治療,約僅10%的人最後需以手術治療。而手術治療方式分為單純神經減壓或神經減壓加上釘棒固定融合,手術方式因人而異,患者的年齡、神經壓迫嚴重程度、勞動需求,以及須考慮脊椎的穩定度等…皆須多方面的考量,而何種手術才是最適當的,門診常見病患與家屬在思考過程中仍有需多疑問與內心的惶恐因而面臨無法選擇的窘境。
此醫病共享決策輔助工具利用文獻收集之醫學證據,以及臨床現有的統計報告,提供病患了解脊椎神經單純減壓與釘棒固定融合的差異處,以及在手術後可能會面臨的問題與考量點,讓病患對此治療有充分的認識,能在考慮各方面條件之下,並且與醫療團隊討論下,提供完整的、清楚的、可了解的治療計畫。
