一、題目重要性：
愛滋病治療在高效抗逆轉錄病毒治療Highly Active Anti-Retroviral Therapy（HAART）問世後，愛滋病議題不再是與死神拔河，而是有效治療與終止傳播。世界衛生組織提出 2020 年達到 90-90-90目標：90%愛滋感染者知道自己感染，90%已知感染者接受藥物治療，90% 接受治療的感染者病毒量受到控制。希望能藉此終結愛滋病流行。
全球動員的熱潮中，臨床人員面臨兩項重要挑戰：
首先，新診斷或未服藥感染者需縮短診斷到投藥的臨床路徑，以期達到於診斷當天開始抗轉錄病毒治療（Same-Day ART）。此外，已服藥感染者需維持高服藥順從性才能達到預期療效。研究指出病人須至少須維持95%以上的服藥順從度才能確保療效以及穩定的病毒控制。
將共享決策導入臨床流程，能強化醫病互動，增加信賴，進而克服未服藥族群對於治療的恐懼，提升已服藥族群對於治療的信心。同時，研發決策輔助工具標準化疾病藥品衛教內容，闡明藥物治療與疾病預後的因果關係，詳述疾病管制署目前第一線的推薦處方：四個單藥錠的藥物特色，包括飲食注意事項、副作用、生活型態調整等影響決策重要因子，再輔以問卷讓病人自身偏好，最後與醫療人員完成共同決策，實現以病人為中心的照護理念。希以此方式克服前述兩項重要挑戰。
二、研發團隊與運作：
雙和醫院愛滋病防治中心，整合感染專科醫師、個管師、臨床藥師等成員，提供病人全方位的跨團隊照護。
臨床藥師將共享決策的理念導入臨床照護流程中，協同團隊中臨床專家，共同探討疾病治療流程與病人需求，訂定決策點。研發輔助工具的過程中，結合實證資源中心進行決策輔助工具的審查。定稿後，由醫品部協助建置診斷碼，將決策輔助工具導入臨床工作過程。
三、實證資料選用及整合過程：
利用實證醫學的手法進行需求調查與修訂決策輔助工具。證據檢索過程設定族群為愛滋病毒感染者，運用PubMed、DynaMed等資料庫，篩選系統性綜論、綜合分析，進行整併。此外，為求切合臨床適用性，考量各國給付規範、醫療資源與文化差異，將2018年台灣愛滋病治療指引列為本次修訂重要參考資料。
四、知識轉譯過程及適用性評估：
為強化決策輔助工具的健康識能，同時兼併臨床需求，臨床醫師、愛滋病個管師及愛滋病患者皆納入輔助工具的測試。專業適切度則由實證資源中心以及臨床醫師共同審查。執行流程在愛滋病防治小組會議中通過後，臨床可行性利用現場實測進行確認。
